DOG TRAINING
Release and Hold Harmless Agreement
I,
____________________________________________________________, being of lawful age,
understand the educational purposes of Gulfstream Rottweiler Club of Greater
Miami and in consideration of being allowed to participate in the dog training
class being provided by Gulfstream Rottweiler Club of Greater Miami, hereby
agree on behalf of myself, my spouse, heirs, legal representatives, assignees,
guests, invitees, and my insurers that: I am fully aware and acknowledge that
training can cause stress in dogs and that the training will be done in the
presence of other persons and animals; I am fully aware of the risks and
dangers involved and hereby accept and assume full responsibility for any and
all risks of bodily injury to myself, my dog, or to any other person and/or
animal which results from attendance and participation in such training; I
hereby release Gulfstream Rottweiler Club and Belen Jesuit Preparatory School
which provides the training grounds, their officers, agents, directors,
employees, representatives, other enrollees, All About Paws, Inc. and their
instructors from any and all liability, fines, or other matters arising from
such attendance or participation; I agree to indemnify, defend and hold
harmless Gulfstream Rottweiler Club of Greater Miami, Inc., Belen Jesuit
Preparatory School, their officers, agents, directors, employees,
representatives, other enrollees, All about Paws, Inc. and their instructor(s)
from any and all claims, demands, expenses, and liability, whether for personal
injury, death, property damage, violations of law which is caused by the
undersigned and which in anyway arises out of or relates to a Gulfstream
Rottweiler Club function or activity.
I have carefully read
and understand the Release and Hold Harmless Agreement and I am executing this
instrument voluntarily.
